We send nurses (RNs and LPNs) and physical therapists to your home to provide health care. We only provide services in Perry County.
To be eligible for these services, you must be live in Perry County Tennessee and be referred by a doctor, podiatrist, or nurse practitioner. You can also be referred to our services if you are being discharged from a hospital or nursing home.
Our nurses can do wound care, obtain lab specimens (we have a partnership with LabCorp), give shots, administer IV therapy, care for ostomies and urinary catheters, drain chest tubes using Pleurex catheters, and perform many other procedures in the comfort of your home.
Our physical therapist can help with walking, home safety, fall prevention, balance issues, and non-pharmacological pain management.
Everyone we serve receives help managing their medications, teaching about your health (including things like how to manage symptoms of your diseases, about your medications, ways to move around safer in your home, and ways to help reduce your pain), and ongoing monitoring by a team of nurses. We help to coordinate care with your doctors and pharmacy to ensure you get the best medical care.
We only accept patients when we believe we can meet their needs. While nursing services are available county-wide, we currently only provide in-home physical therapy to residents of Linden.
